CAROMAS HEALTHCARE SERVICES, LLC: PRIMARY CARE CLINIC PRIVACY POLICY
Effective Date: September 3, 2026
The privacy and security of your information are very important to CaroMas Healthcare Services, LLC (“CaroMas”). This Privacy Policy explains how we collect, use, disclose, and safeguard the personal and health information you provide to us through our website, patient portal, telehealth services, electronic communications, and other services.
CaroMas Healthcare Services LLC respects your privacy. This policy explains how we collect, use, disclose, and protect information obtained through our website, patient portal, electronic forms, telehealth services, text messaging, and related online services. It does not replace our HIPAA Notice of Privacy Practices, which explains how we use and disclose protected health information (PHI).
1. Information We Collect
We may collect information you provide, including your name, date of birth, address, email address, telephone number, insurance and billing information, appointment information, and health information. We may also automatically collect limited technical information, such as IP address, browser or device type, pages visited, referring website, and access date and time.
2. How We Use and Disclose Information
We may use or disclose information as permitted or required by HIPAA, the Texas Medical Records Privacy Act, and other applicable laws for treatment, payment, healthcare operations, care coordination, appointment or result notifications, billing, website operation, patient safety, legal compliance, and public health purposes. Uses or disclosures of PHI not otherwise permitted by law generally require your written authorization. We do not sell PHI.
3. Cookies, Vendors, and Website Security
We may use necessary cookies and similar technologies to operate, secure, and improve our website. Vendors that handle PHI for us must protect it as required by law and, when applicable, sign a HIPAA business associate agreement. We do not permit tracking technologies to collect or disclose PHI unless legally authorized and appropriately safeguarded. Links to third-party websites are governed by those parties' privacy policies.
4. Text Messaging and Electronic Communications
Your phone number and SMS consent will never be shared or sold to any third party or affiliates for marketing purposes. With your consent, we may send SMS or MMS communications concerning appointment reminders, scheduling updates, billing notices, and other healthcare-related administrative messages. Message frequency varies, and message and data rates may apply. Reply STOP to opt-out text messages or HELP for assistance. Consent to texts is not required to receive care. Standard email and text messages may not be encrypted; avoid sending urgent or highly sensitive information through unsecured channels. Call 911 for a medical emergency. You are responsible for informing us if your telephone number changes or is reassigned
5. Email and Website Forms
Ordinary email and unsecured website forms may not be encrypted. Please avoid submitting sensitive medical information through an unsecured communication channel. If we provide a secure patient portal or secure messaging platform, use that service for non-emergency clinical communications whenever possible. Submitting a website form does not establish a provider-patient relationship. A provider-patient relationship is established only after CaroMas accepts you as a patient and completes the appropriate clinical and administrative processes.
6. Website Monitoring and Security
We may monitor network traffic, access attempts, system performance, and other activity to: Maintain system availability; Identify unauthorized access; Prevent fraudulent or malicious activity; Investigate security incidents; and protect our systems, information, patients, and workforce. Monitoring will be performed in accordance with applicable law and will not authorize uses or disclosures of PHI that are otherwise prohibited.
7. Protecting Your Information
We use reasonable administrative, physical, and technical safeguards to protect personal information and PHI. No electronic transmission or storage method is completely secure. Therefore, although we take reasonable measures to protect information, we cannot guarantee absolute security. If a breach of unsecured PHI or other legally protected information occurs, we will provide notification as required by applicable law
8. Your Privacy Rights
Subject to applicable law, you may request access to or a copy of your records; request correction of your records; request confidential communications or certain restrictions; receive an accounting of certain disclosures; obtain a paper or electronic copy of our HIPAA Notice of Privacy Practices; revoke a prior authorization in writing; and file a complaint without retaliation. Some requests may be limited by law, and we may verify your identity before responding. Submit a privacy complaint without retaliation. Texas consumer privacy rights may also apply to certain personal data; HIPAA-regulated information and certain entities or activities may be exempt. To exercise a privacy right, submit a written request using the contact information provided below. We may need to verify your identity and authority before completing the request.
9. Children's Privacy
Our general website is not directed to children under 13, and we do not knowingly collect personal information online directly from a child under 13 without legally valid parent or guardian consent. Information concerning minors who receive healthcare is handled under HIPAA and applicable Texas law.
10. Third-Party Websites and Services
Our website may include links to third-party websites, portals, applications, payment services, or resources. CaroMas does not control and is not responsible for the privacy, security, availability, or content of third-party services. A link does not constitute an endorsement. You should review the privacy policies and terms of any third-party service before providing information.
11. Information Retention
We retain personal information and medical records for the periods required or permitted by federal and Texas law and as reasonably necessary for treatment, payment, healthcare operations, legal compliance, dispute resolution, and security purposes. When information is no longer required, we take reasonable measures to securely destroy or de-identify it, subject to applicable record-retention requirements.
12. Changes to This Privacy Policy
We may revise this Privacy Policy periodically to reflect changes in our Services, technology, privacy practices, or legal obligations. The revised policy will be posted on our website with an updated effective date. Changes will apply prospectively unless otherwise required or permitted by law. Material changes involving PHI will also be addressed through our Notice of Privacy Practices when required.
